Online Locate Request BLUE STAKES OF UTAH 811
This online form can be used by anyone from a homeowner to a professional excavator
to request to have utility lines located and marked at a single address.
The form is designed for simple requests only and should NOT be used for multiple
addresses
or intersections. Larger or more complex location requests can be processed online
using
other web applications after training has been completed. Click

Emergency tickets or tickets that require a
Meet
with the locators must be processed by phone through the Blue
Stakes
Notification Center and cannot be submitted online.
To use this program you must adhere to ALL of the following:
A valid e-mail address must be provided. Blue
Stakes will send you a copy of the
locate request when it has been processed and sent to the member facility operators.
Your request is not valid until you receive the
locate request number which will
be delivered to you by e-mail from noreply@bluestakes.org. Ensure that you
can
receive email from this address and that it isn't delivered into your
junk/spam
folder. Please do not reply to the confirmation message.
Requests received by Blue Stakes after 3:00
p.m. may be processed on the next business
day.
Markings should be completed by the member facility operators
within three (3) business days
after you receive a copy of your locate request by e-mail.
The member facility operators will mark the lines that
they own and maintain within the described excavation area.
No private lines will be marked.
The party doing the actual excavation (property
owner and/or contractor) must create a locate
request in their name.
The exact address of the property
must be provided and posted on site.Do not list an
intersection
or lot number. Do not include marking instructions for other
properties.
The area to be marked must be accessible (free
from animals and locked gates) for
three (3) business days from the date you receive your location request by
e-mail.
Excavation sites that require additional
marking instructions MUST be processed by
calling 811 or using other web applications after training has been
completed

Blue Stakes will only notify participating member
utilities of
the proposed excavation.
If you are aware of additional underground utilities in the vicinity of your
proposed
excavation that are not listed on the e-mail confirmation from Blue Stakes, it
is
your responsibility to contact those entities directly.
If you do not receive an e-mail confirmation from
Blue Stakes
within 24 business hours,
please re-submit the request or call 811.
Excavation activity must not begin until after the
legal date
and time referenced on
the e-mail confirmation you receive from Blue Stakes of Utah.
Your locate request will expire 21 calendar days
after it is
created or when the marks
are no longer visible, whichever comes first.
If you need to dig beyond the expiration date of
the locate
request (listed on your request
as the "Good Thru" date), you must contact Blue Stakes again and create another
locate
request no later than two business days before the expiration date. This date
is
listed on your request as the "Update By" date.
Power operated equipment is not permitted within 24
inches on
either side of locate markings.
Use hand tools only within this area!
Participating member utilities are not responsible
for marking
private underground lines.
The address must be posted and open access to the
property must
be provided (no locked
gates or uncontained animals).
Please keep your ticket number as proof of
contacting Blue
Stakes; there may be a research
fee to find lost ticket numbers.
